A Glencoe man was in Lake County court Thursday on charges of reckless homicide stemming from a fatal accident Jan. 13 in Lincolnshire.

John Warsaw, 63, of 1194 Carol Lane, was charged with reckless homicide and two counts of aggravated driving under the influence causing death or great bodily harm, police said. A passenger in the vehicle he struck head-on, Shruti Phandis, 26, of Wheeling, died almost five days after being transported to Advocate Condell Medical Center, Libertyville. She had just been married the day before the crash.

Warsaw is out of jail, paying $50,000 on the $500,000 bond.

Both Lincolnshire and Buffalo Grove police received numerous 911 calls just before 9 p.m. on the day of the crash by people reporting a reckless driver who was in the wrong lane of traffic. Warsaw, driving a Honda CRV, apparently went into a ditch and then a median, where he took out a street sign on Route 22 in Buffalo Grove.

He continued east on Route 22 and then south on Route 21. Just south of Aptakisic Road he crossed the center line and was driving southbound in the northbound lanes. The Honda collided head-on with a Toyota Corolla and pushed it into a Jeep Grand Cherokee behind it. Phandis was a front seat passenger in the Toyota. Other drivers and passengers were treated and released from local hospitals.

The Major Crash Assistance Team of Lake County assisted in the crash investigation. On Wednesday the Lake County State’s Attorney’s Office approved the charges and an arrest warrant was issued. Warsaw surrendered himself that afternoon and was transported to Lake County Jail to await the court hearing Thursday.
